Three selected randomized reports cover cardiovascular events in older adults with type 2 diabetes, glycemic outcomes in youths, and glycemic and kidney measures in adults with chronic kidney disease. This overview does not cover the full AWARD program or all later analyses.123

REWIND: cardiovascular outcomes

Design

A blinded randomized placebo-controlled trial followed participants for a median of 5.4 years.1

Population

The trial enrolled 9,901 adults aged at least 50 with type 2 diabetes and cardiovascular disease or risk factors.1

Outcomes reported

The primary cardiovascular composite occurred in 12.0% with dulaglutide versus 13.4% with placebo (hazard ratio 0.88; 95% CI 0.79–0.99). Gastrointestinal adverse events were more frequent with dulaglutide.1

Limitations

The composite combines different outcomes and does not imply improvement in every component. All-cause mortality was not significantly different; these results do not establish effects in people without type 2 diabetes.1

AWARD-PEDS: glycemic control in youths

Design

A blinded randomized placebo-controlled study evaluated a 26-week primary endpoint, followed by an open-label extension.2

Population

The trial randomized 154 participants aged 10 to under 18 with type 2 diabetes.2

Outcomes reported

Glycated hemoglobin decreased in the dulaglutide groups and increased with placebo. BMI change did not differ between groups; gastrointestinal adverse events were more frequent with dulaglutide.2

Limitations

The primary outcome was glycemic control, not cardiovascular events or long-term complications. The short blinded period limits longer-term conclusions; PubMed links subsequent correspondence.2

AWARD-7: diabetes with chronic kidney disease

Design

An open-label randomized trial compared dulaglutide with insulin glargine, with insulin lispro in all groups, over 52 weeks.3

Population

The study randomized 577 adults with type 2 diabetes and stage 3–4 chronic kidney disease.3

Outcomes reported

Dulaglutide met the prespecified noninferiority criterion for glycated hemoglobin change at 26 weeks. At 52 weeks, estimated filtration rates were higher with dulaglutide; nausea and diarrhea were more frequent.3

Limitations

Kidney measures were secondary outcomes. This trial cannot establish a general prevention claim for kidney failure or isolate the effects of background insulin treatment.3
