NCT03574597 ↗

Semaglutide

What was tested?

Subcutaneous semaglutide versus placebo in people with established cardiovascular disease. ↗

Registered primary question: Time to the first cardiovascular death, nonfatal heart attack or nonfatal stroke; the registry observation window extended to 240 weeks. ↗

What the reviewed report found

Journal year 2023 · Article summary reviewed 2026-08-18

A primary cardiovascular event occurred in 6.5% of the semaglutide group and 8.0% of the placebo group; the reported hazard ratio was 0.80. Permanent discontinuation because of adverse events occurred in 16.6% and 8.2%, respectively.↗

Keep in context: The report gives a mean follow-up of 39.8 months. That observed follow-up differs from the maximum registry observation window. ↗↗

SELECT cardiovascular-outcomes trial

Design in the report
A multicenter, double-blind, randomized, placebo-controlled, event-driven superiority trial assessed a composite of cardiovascular death, nonfatal myocardial infarction, or nonfatal stroke.↗
Participants in the report
The trial enrolled 17,604 adults age 45 or older with established cardiovascular disease and overweight or obesity, but no history of diabetes.↗
Limitations
The sponsor-funded findings apply to people with preexisting cardiovascular disease and without diabetes; they do not answer primary prevention for lower-risk populations.↗
